How the work moves

  1. Report the finding

Identify the process, describe what went wrong and rate its severity. The investigator starts with a record of the observed issue.

  2. Investigate the cause

Record the investigation and the corrective action taken. Submit it for verification when a colleague can check the result.

  3. Verify the correction

An independent verifier records the outcome. A successful check closes the action. When the evidence needs more work, the finding returns to investigation.
